Improving `Custom conditions in "exports"` solutionOpenIssue

Hey @colinhacks, first of all, great work with the repo and article! It's a god send! I will focus my feedback on the recommended solution of using custom conditions. 1) In article you mention that should be first export in but this is not the case in the repo. 2) I think this repository could use more "real world" structure -- moving into src directory. I didn't realize that it requires a tweak in package.json exports and I wasted some time on this. If you agree I can make a quick PR improving this. Anyway, amazing work overall 👍
